A cock ring should be put on while you’re flaccid. Some go over the base of the shaft, and some are meant to include the testicles, which are pulled through the ring so it is at the very base of your genitals. 

Some are stretchy silicone–which are more comfortable to wear, and more beginner-friendly–while others are made of metal, which are more intense to the wearer. 

I suggest this model for beginners who are looking for a basic ring. I own the vibrating version (which obviously has benefits for your partner during sex, as well) and had good feedback from my partner on the ring itself.

Basically, yes, the idea is to hold blood in the penis by using the ring. This is probably a terrible analogy but I think of a balloon. You can fill it with air and you have your erection but if you fill it with the same amount of air and squeeze the base, there will more pressure built up, resulting in harder, stronger erections. When you ejaculate with a cock ring on, there is usually a lot more pressure and hence, a stronger ejaculation. You remove the cock ring after ejaculation when you’re flaccid again.
